News from the Lake County Health Department
The Lake County Health Department would like your help in completing a community health assessment survey. We want to know your perspective about the health and wellness of your community. The information you provide will contribute to prioritizing the health concerns of Flathead Reservation and Lake County residents.
The information we collect from your anonymous responses will be compiled and ranked, and the results will be published later this year. The survey was developed with the assistance and input of community stakeholders and should take approximately 15-20 minutes to complete.
The survey is available on-line at the following link: https://www.surveymonkey.com/r/LakeCountyCommunityHealthAssessment or can be accessed by scanning the accompanying QR code.
There are also hard copies available at the health department, St. Luke Community Hospital, St. Joseph Medical Center, Polson Health, the CSKT Social Services office, Lake County Community Development, and other sites around the county. You may request a paper survey by calling LCPH at 406-883-7288. Surveys must be completed by Aug. 15.
