Care coordinator

CARE COORDINATOR
TRIBAL HEALTH DEPARTMENT
The successful applicant must possess Post- secondary education in mental health related field or 2 years' related experience and/or training; or equivalent combination of education and experience. Experience in grant funded activities including fulfilling objectives and completing progress reports and financial reports. Current, unrestricted Registered Nurse license from the State of Montana or other state. Graduate from an accredited Bachelor of Science in Nursing (BSN) program, with two years of nursing experience, with at least one (1) year of ambulatory care nursing experience preferred. OR Currently enrolled in a BSN program. Five (5) years of nursing experience, with at least one (1) year of ambulatory care nursing experience preferred. Basic Life Support certification. Advanced Cardiac Life Support preferred. Certification in Care Coordination preferred, required within three (3) years of employment. Must maintain proper licensure/ certification and registration; if required. Must possess a valid driver's license. CDL driver

Facilities/transportation coordinator

FACILITIES/ TRANSPORTATION COORDINATOR– ECS Monday – Friday (8 – 4:30)
   
The successful applicant must possess a High School diploma or equivalent. Four (4) years' experience acquiring basic knowledge and skills in repair and maintenance of equipment, furniture, fixtures, facilities systems, grounds keeping, and structures. Minimum of five (5) years' working in a supply/inventory setting as a procurement person. Knowledge of school bus transportation and safety systems. Two (2) years of documented supervisory experience with adults. Must have or obtain a commercial driver's license (CDL) within 3 months of hire. Employment preference given to current or former Head Start/Early Head Start parents. Must provide documentation. Health Requirements: Current Physical Exam or ability to obtain Physical Exam upon hire. Current on required immunizations or ability to become current upon hire. Verification of recent TB screening or ability to complete required screen. Must pass drug test and complete a Criminal Record Check including finger- printing, (no violence against children or individuals, no domestic violence, no sexual abuse or violent acts against children or any person) prior to employment and every 2 years thereafter; subject to random drug testing through the Confederated Salish and Kootenai Tribes' Personnel Department. Possess and maintain a current Montana driver's license and be insurable under the Tribes' fleet policy. May be employed under a contract. Salary range $16.98 to $19.74 per hour, plus benefits. HEALTH & WELLNESS DIVISION DIRECTOR DEPUTY II, TRIBAL HEALTH DEPARTMENT
    The successful applicant must have a Bachelor's degree in a health related, communication or human services field, with 5 years' experience in administrative experience, including supervision of staff and managing budgets. Must have professional experience in developing, implementing and evaluating health prevention and intervention services according to best practices for Native Americans. Must possess a vailid driver's license. Must adhere to Tribal Resolutions #94-74 and #06-48 regarding employee conflict of interest and employee standards of conduct for Tribal employees.  Must meet all annual training and system requirements to maintain access to required computer systems. Must maintain proper licensure/ certification and registration; if required. Must participate in and support HIPAA training, as needed and required. Must adhere to CSKT social media policy and other CSKT Tribal policies regarding professional conduct. Will be required to participate in pre-hire drug test and random drug tests thereafter. Must pass a background and suitability check according to Public Law 101-630; the Indian Child Protection and Family Violence Prevention Act. Must adhere to all Tribal Health directives and policies.  May be required to attend evening meetings to participate in health services outreach and or health fairs to reach designated population, must possess a valid driver's license, complete a supplemental questionnaire for background investigations (see Personnel Department).  All applicants must submit a Tribal application and copy of academic transcript/training certificate, proof of enrollment in a federally recognized Tribe if other than CSKT and if claiming veteran's preference, a copy of DD214 must be submitted.
