   The successful applicant must possess a high school diploma or equivalent. Two (2) years’ experience in accounts receivable and payable. Three (3) years of clerical support experience in an office environment. One (1) year of demonstrated computer skills in the use of word processing, spreadsheets, e-mail, internet, and databases. Employment preference given to current or former Head Start/Early Head Start parents. Must provide documentation. Possess current CPR and First Aid certification with Infant/Child endorsement or be willing to obtain upon hire. Case worker

St. Luke Community Healthcare is currently hiring a Case Manager/Social Worker with the desire to improve the quality of care and meet the psychosocial needs of our patients. In collaboration with a multi-disciplinary team, the Case Manager/Social Worker has input in the development of the plan of care for those patients requiring social work intervention. Responsible for providing medical and social service case management, psychosocial assessments, planning, interventions and follow-up for patients and family members across the continuum of primary care, and working side-by-side with patients to address social determinants of health and barriers they are facing which limits their ability to obtain whole health and wellness.
